PARENT CONSENT AND WAIVER FORM

for Student-Participants in 2011 Area Level Schools Press Conference

In consideration of the forthcoming competition for Area Level Presscon 2011 to be held at Sibonga National High School, Sibonga, Cebu, Guindaruhan National High School (GNHS) has 12  qualified contestants for said competition in various English categories, namely, editorial writing, news writing, feature writing, sports writing, copyreading and headline writing, photojournalism, editorial cartooning and radio broadcasting.

Through this “Parent Consent and Waiver Form,” it is clearly understood that the undersigned parents or guardians of the student-participants agree in accordance with the policy of the school, read this waiver-content, and fill out the form with understanding and scrutiny as follows:

As a parent/guardian of the student-participant, I hereby give my consent, and acknowledge by my signature that my son/daughter                                                      of Year level and Section                   will join the Area Level Schools Press Conference on October 10-11, 2011 at Sibonga National High School, Sibonga, Cebu.

Based upon the information given to me by my son/daughter, due to the budget constraints experienced by the PTA, for the meantime, aside from food, I will give him/her pocket money. Other payments (registration fees, membership fees, fares, incidental expenses, etc.) will be shouldered by the accompanying advisers through PTA funds.

I am aware of the usual risks and dangers inherent in presscon-participation; thus, accidents can occur with or without any fault on either the part of the student, or the school board or its teachers, or the facility where the presscon activity is taking place. By allowing my child to participate in this activity, I accept the risk or any untoward incident, and conform to the school rules and regulations  that this activity is suitable for my son or daughter in order to enhance his/her skills in journalistic writing.

In signing the Consent and Waiver, I am not relying on any oral or written representation or statements made by the School Board and its teaching personnel, or authorized adviser to induce me to permit my student-participant to the trip intended for presscon contest, other than those set out in this Consent and Waiver.
